7.1 Document header

A sample syntax for the header of a document using:

1. html.sty for the L^ATEX2HTML hypertext extensions,
2. makeidx.sty for index generation using LaTeX \index tags, and
3. epsf.sty for inserting Postscript files as graphics.

is:

\documentclass{article}
\usepackage{html,makeidx,epsf}

Do not use the older L^ATEX \documentstyle command to call up style files. Many bugs emerge from L^ATEX2HTML when this is done!
